Thank you for choosing Tschetter & Hohm Clinic, P.C. Our goal is to provide a personalized healthcare experience by using traditional diagnostic processes enhanced with modern-day equipment and technologies.
Your First Visit
Prior to your first visit, please print and complete our new patient paperwork. If you are unable to do this before your appointment, please arrive 15 minutes early so that you can complete it in the office.
*Note: Messages sent using this form are NOT confidential. Avoid sending highly confidential information or private information through this system.
We see patients on an appointment basis; this is not a walk-in clinic. To schedule an appointment, please call 605-352-8767 or 800-514-1447. Please identify the physician you would like to see and let the receptionist know during the call.
We strive to stay on time throughout the day because we value your time. Occasionally, unforeseen delays can interrupt our day. Please be patient and know that we are doing our best to stick to our schedule.
If you are unable to attend your appointment, please notify the office as soon as possible. This way, we can schedule another patient for that time and reschedule you for another appointment.
Insurance and Billing
We accept most major insurances. You are responsible for your copayment or coinsurance at the time of service. We accept cash, checks, debit, and credit cards for payments. If you have questions about payment, please call 605-352-8767 to speak to a Patient Accounts Representative.
Your Patient Rights and Protections Against Surprise Medical Bills
To help patients better plan for health care expenses, the No Surprises Act, which went into effect January 1, 2022, health care providers and facilities must provide a good-faith estimate of expected charges to uninsured consumers, or to insured consumers who choose not to have their health plan help cover their health care costs.
The good-faith estimate is based on information known at the time the estimate was created, therefore any unknown or unexpected costs such as Laboratory and Radiology tests your provider may order during your office visit are not included in this estimate. A good-faith estimated for other services and procedures can be provided to you upon request.
When you schedule your appointment and indicate no insurance or a desire to not have insurance cover the cost of the scheduled appointment, our registration staff will ask if you’d like us to provide an estimate. Cost estimates are also available through a convenient online calculator: www.huronregional.org/pricetransparency.
Just visit this link and search for the service – you can even save and print the estimate.
For questions about your right to a Good Faith Estimate visit www.cms.gov/nosuprises or call 1-800-985-3059.
For questions about charges or billing or help in estimating out-of-pocket costs, visit our price estimator or call 605-352-8767.
